PHCY 4070 - Transitional Care

This one-month advanced pharmacy practice experience (APPE) is an elective patient care component of experiential learning. This rotation is an introduction to the growing emphasis on transitions in care that is associated with disease management. This experience is designed to enhance the student pharmacist’s knowledge and ability to individualize didactic course material to a specific patient. The student will be responsible for applying appropriate pharmacologic therapy specific to patient diseases encountered and enhancing communication skills through patient and health care provider education and facilitation of continuity of care.

This course may not be repeated.
