IDE 45070 - Transforming Healthcare Delivery

This is a face to face clinical rotation receiving 6 credit hours. The student should function with increased autonomy in patient care and must demonstrate competency in several of the Entrustable Professional Activities required for graduation. The University of Tennessee Medical Center has been engaged in a decade long commitment to transforming healthcare delivery by developing a clinically integrated network. Clinical Integration (CI) is the coordination of health services across the continuum of care including disease prevention and wellness promotion, management of the acute care episode, chronic disease and medication optimization, and developing effective provider-payor relationships. The goal of CI is to improve the quality of care and health outcomes of the population, while containing costs of care delivery. By spending time with patients, practitioners, and healthcare leaders in each phase of the inpatient and outpatient care continuum, the student will gain understanding of the economics and management of value-based care delivery. Along with patient care responsibilities the student will interact with senior executives and key members of the system management team. A designated CI leader will act as a mentor to the student. The student should expect to attend management meetings and leadership rounds, maintain a journal of experiences, and complete a specific CI project under the guidance of the mentor.
