HOPR 832  - Mentored Practicum in Population Health

This advanced course on population health reviews the factors that are contributing to worsening health
status among the US population and presents the reasons why our healthcare system in its current form
has limited effectiveness in addressing contemporary health problems. Taught entirely on-line, this
practicum focuses on the social determinants of health and subsequently emphasizes the importance of
addressing these determinants as a means of improving a population’s health. It describes the advantages
of the population health assessment process compared to the traditional comprehensive health needs
assessment. The course involves a series of hands-on activities that illustrate the techniques employed
in performing a population health assessment. The exercises are designed to demonstrate the ways in
which a population health approach can be used to improve community health and illustrates the use of
cross-sector collaboration for collective impact on community health.
